Hole in disk of Serpant Star - What to do?

So I finally made the move back to my 75g after moving into a 30, moving cities, etc... What a pain.

My green/white serpant star which is the first critter we bought when we set the tank up, appears to have a fleshy hole in the top of his central disc. Should I dispose of him or move him into the 75g tank?

I would transfer him, and give him a chance. Watch him over a few days, and make sure he is still moving. I have a striped serpent star that lost a leg and completely regrew it. If his mouth is still intact, he maybe will have a chance.
